How to Request a Player Refund

SMHA Refund Policy:

* Before Sept 1, 2012: Full refund less a mandatory $25 Administration fee 

* Sept 2-30, 2012: Full refund less a $125.00 Administration fee  

* Oct 1 – Nov 30, 2012: Refund of 50% of the membership fee

* After Nov 30, 2012: No refunds for any reason

All refunds will be processed according to the above procedures using the date the request was received in writing or by email to the SMHA Registrar as the effective date of the request.
