All volunteers within SMHA must have a completed Police Record Check dated within the previous 24 months.  This includes, coaches, managers, trainers and all on-ice helpers. PRC's are free for any volunteer within a non-profit association, provided a volunteer letter signed by the association president is used. To obtain a form, please contact: 
 vpsupportservices@stittsvilleminorhockey.com

Forms can be dropped off at:

211 Huntmar Drive (Kanata) which is open from 7 am - 9 pm Monday to Friday.  If those hours are not convenient, then the Elgin St Police Station is open 24/7 Monday to Sunday.
