OLD NEW PLAYERS

For players never having played in another association previously:
Download and complete the ***REQUEST FOR REGISTRATION FORM***. This completed form needs to be accompanied by the following documentation:
  • Proof of Age - Photocopy of birth certificate and/or passport. A Health Card is not acceptable.
  • Proof of Residency - Natural Gas, City of Ottawa Water invoice showing your current address with the dollar amount blacked out, home lease agreement for renters, or agreement of purchase or sale. Driver's License and Health Cards are not acceptable.
 
Options for documentation verification:
  1. Email all paperwork to registrar@stittsvilleminorhockey.com
  2. Attend with completed paperwork on Wednesday June 26th 2019 at 6:30pm to 8:30pm the walk-in IP and Half Ice Novice information night. It will be held at Browns Your Independent Grocer upstairs in the community room. Brown's is located at 1251 Stittsville Main St, Stittsville.
  3. Deliver your paperwork to the Johnny Leroux Community Centre (a.k.a. Stittsville Community Centre) and place in the SMHA mailbox located at the arena across from the Referee's Room.  The arena is located at 10 Warner Colpitts Lane.  Please note JLA is not consistently open in the summer evenings due to a lack of activity. The best times for drop off are between 7am and 5pm.
Once the above is verified, a Hockey Canada # will be provided with instructions to proceed with online registration.

Your registration with SMHA cannot be confirmed until all forms are completed correctly, online registration is complete and payment is received.  The recommended method is attend an in-person information evening with the REQUEST FOR REGISTRATION FORM so they can be checked for completeness and accuracy. If you mail in/dropoff your application, we cannot be responsible for mail delivery issues or incomplete forms which will be not be processed.

Respect In Sport training - This season before your child's registration is finalized, one parent must complete the RIS online course.  Further information can be found here.

Please note that making an application through either method is NOT a guarantee that your child will be registered to play minor hockey in 2021/22.  Players who have registered on or before July 31, 2021 will be guaranteed a spot on a team assuming there are enough players registered in the division to have a team.  All players who register on or after August 1, 2021 will be placed on a wait list and only placed on a team should there be space available.
